The premise of the game (GTGFTS) is simple: I’ll post a scoreboard and you guys simply identify the game depicted. In the past, I don’t know if I’ve ever completely stumped you (some are easier than others).

Maybe he was issued Rick Wise’s old jersey (was he sent down to the minors beforehand?) ?
Hi. Looks like Cardinals at Phillies, September 10, 1964. The Phillies won 5-1, with four unearned runs thanks to six Cardinals errors. Judging from the runner’s gait, that’s Phils RF Johnny Callison rounding the bases after hitting a three-run dinger off of Ray Sadecki.
After this win, the Phillies held a 6-game lead in the NL standings over the Cardinals, Reds, and Giants. Three weeks later, after a 10-game losing streak in which they were swept by the Reds, (Milwaukee) Braves, and these same Cardinals, the Phillies found themselves in third place behind St. Louis and Cincinnati. They avenged themselves—against the Reds, anyway, by sweeping a two-game set to finish the season, while the Cardinals ended the season by whomping the Mets behind Bob Gibson to clinch the 1964 National League pennant on October 4, 1964, the day after I was born.
